The Process of Obtaining a Physician Certification for a Medical Marijuana Card
Step 1: Call for an appointment
♦ When you call for an appointment, our knowledgeable staff will answer any questions you may have, advise you on what forms to bring, and set a specific time and date for you to meet the doctor.
Step 2: Complete the necessary forms
♦ You can download the office forms here and bring them to your visit, or you can come in a little early and fill them out once you arrive at our office on the day of your visit.

Step 3: Come in for your first visit to complete a medical history and get a physical exam
♦ New law (SB-109) states that the physician must have previously seen the patient and personally completed a physical exam before making a recommendation for a Medical Marijuana card.
♦ This visit will last 30-45 minutes as the doctor will do a full history relevant to your medical marijuana condition.
♦ At this appointment, you may ask the doctor about any areas of health that are of concern.
Step 4: Come back for your Medical Marijuana recommendation and get your application notarized
♦ At this visit the doctor will evaluate your qualifications for a Medical Marijuana card, and if you qualify, will give you a recommendation.
♦ Get the application form notarized, which we will happily do in our office at no extra cost. We have a Notary Public on staff.
Step 5: Get the Medical Marijuana card
♦ You mail in the recommendation within 60 days and your card will be mailed to you once everything is processed by the state.
